I would like to become a member of the International Normobaric Association of Doktor Pokrywka. I declare that I know the Articles of association, as well as its goals. I will abide by the laws, pay the membership fee on time and actively participate in the operations of this organization.

I will pay the membership fee of 1500 zł for the current calendar year no later than one month after being approved as a member of the Association via bank transfer to the association's bank account: PL 68 1240 4025 1111 0010 9414 0849 (SWIFT/BIC: PKOPPLPW).
